Functional Vision Assessment, Learning Media Assessment, and Learning Media Plan Updates

The Functional Vision Assessment and Learning Media Assessment have been separated within the Assessments drop-down options. Also, the Learning Media Plan (LMP) document has been approved and is available as a fillable PDF. The LMP shall be uploaded into Supplemental Documents for students with visual impairments including blindness. Monitoring and Compliance Branch has updated some of the printable forms.

On February 8, 2024, the Monitoring and Compliance Branch (MAC) issued a memo regarding the updated form for the Resolution Meeting Summary and Agreement. On February 15, 2024, the Monitoring and Compliance Branch (MAC) issued a memo regarding filing a Special Education State Written Complaint and an ImpartialDue Process Hearing.All the forms were updated to highlight the required elements and provide them in an efficient and user-friendly format. Copies of these forms and additional information are also available on the Department’s Dispute Resolution Website.
